SUMMARY: Care Coordinator is a position that requires organizational knowledge, and ability
to communicate with the agencies pending patient referrals to ensure services
are coordinated and expedited under Medicare Guidelines in a timely manner to
ensure optimal patient care. 
ESSENTIAL DUTIES AND RESPONSIBILITIES include
the following. Other duties may be assigned as necessary.
Coordinate Care for home care patients by assigned team using the Welcome Call Script
Work in collaboration of the scheduling department and inside your team to ensure
timely Starts of Care and Resumption of Care for all patients
Review and work on any and all Non Admit Detail notes on your assigned teams
Ability to learn and retain medical terminology and protocols.
Serve as the liaison between company and patient/caregivers
Track and follow up with all patients who are pending start of care
Track and follow up with all patients who are currently in hospitals or facilities
Coordinate private duty homecare services for potential patients
Coordinate with Liaisons/Sales Team on all patient refusals for care
Position may require weekend rotation.
All other duties assigned by Management
QUALIFICATION REQUIREMENTS:
To perform this job successfully, an individual must be able to perform each
essential duty satisfactorily. The requirements listed below are representative
of the knowledge, skill, and/or ability required. Reasonable accommodations may

Familiarity with medical terminology is preferred
Excellent customer service and verbal communication skills
Excellent organizational skills
Ability to work well in a team environment
Must have accurate data entry skills
Position may require weekend rotation
Proficient in Microsoft Office suite, CRM software, Homecare Homebase software
EDUCATION and/or EXPERIENCE:
Associates Degree, preferredResidential Home Health, Residential Hospice, and Healthy Living Medical Supply are all part of the Residential Continuum of Care.
Minimum of three (3) year
