Nurse Practitioner/Physician Assistant needed for medical office that serves seniors in Metro Detroit area. Home care patient practice. Responsibilities include patient diagnosis, treatment, and care planning.

Reports to: Physician Owner

Salary: NP = $87,500 – $90,000, no benefits

PA = $92,000 – $95,000, no benefits

Travel reimbursement (IRS rate)

Work Days: Monday – Friday, 8:00 am to 5:30 pm

Start date: ASAP

Duration: Long term

Location: Metro Detroit

Minimum Requirements:

Required Certifications/Licensures: PA and/ or FNP state license, CPR or BLS Certification, and ACLS, PALS preferred.

Experience: Minimum of two years in primary care, family practice, internal medicine, disease management, gerontology.

Essential Function:

Practices in Home Based Primary Care.

Academically and clinically prepared to perform a broad spectrum of patient care services at the expert level.

Practices autonomously with consultation as necessary with licensed Medical Staff.

Provides primary care to adults in the area of their clinical specialization which includes assessment, diagnosis, treatment and outcome measures of healthcare problems.

Responsible for initiating and adjusting therapeutic regimens, performs histories and physicals, orders diagnostic and therapeutic tests/studies, initiates consults and referrals, and prescribes medications/supplies in accordance with State regulations and scope of practice.

Documents vital signs, assessments, notes, etc., using the electronic medical record or other computer application.

Performs in-depth health risk assessments on all assigned members, in skilled nursing facilities (SNF), nursing care facilities or in members’ homes.

May be responsible for care plans and treatment for members in medical management settings.

Works collaboratively with physicians, social workers, family, key care givers and any appropriate ancillary medical personnel.

Reviews and interprets the medical and social history of members.

Performs appropriate medical screening procedures for members.

Interprets laboratory data, orders additional laboratory investigations when indicated and ensures appropriate follow-up when assigned to medical management services.

13. Provides prescriptions and administers medications when assigned to medical management services.

14. Ability to write clear, concise reports and the ability to formulate and implement an effective treatment plan.

15. Possess excellent oral English communication skills.

You must pass drug, alcohol, employment and criminal background checks as a condition of employment.

17. Others duties as assigned.

You must have reliable transportation and valid driver’s license.
